Client was monitored and supported for fully participating in classroom activities. Client was reported to be having a good morning by his teacher. Client displayed positive behaviors, despite distractions around him. Client received permission from his teacher to take a break from the classroom. Client displayed excited behaviors as he walked down the hallway with QMHP, as demonstrated by him laughing and expressing cheerfulness. Client did not have to be redirected for any negative behaviors at this time. (PM)Client appeared to be motivated by positive encouragement while being praised for his hardwork. Client said thank you and prepared for lunch. Client was polite to staff and exhibited good manners by saying "thank you" and "excuse me". Client expressed positive emotions. Client was monitored and supported for his cooperation and following lunch rules. Client walked to his session area safely and smoothly. Client was excited to see his counselor and happy to join his therapy session. Client introduced himself and was very welcoming and comforting to other group members. Client was able to utilize his break to draw and engage in reflective thinking. Client is seen expressing positive thoughts and being a team player. Client has already added value to the group by being a leader and sharing his knowledge with others. Client is observed adapting to new environment and making good choices. Client interacted well during group and demonstrated good team work skills by sharing and taking turns. Client was observed exhibiting positive behaviors. Client was praised for not running down the hallway when returning to class. Client utilized acquired skills learned in therapeutic day treatment to focus on his work and make good choices. Client was observed displaying calm, non-aggressive behaviors around peers and authority. Client appeared focused and undistracted as he listened to his teacher review the next lesson. Client was supported with being in class and behaving himself around others. Client did a good job staying seated and went into dismissal with a positive attitude. His teacher did not report any other feedback regarding his choices and it is evident client achieved his goals.
